Therapy intake form
Sex therapy or coaching helps with a variety of different issues. My therapy combines psychological approaches with physical sex work to achieve real world outcomes. I offer separate psychology only services, but this form is for my combined service which includes psychotherapy, tantra and sex work depending on the service level you choose. Please ask me if you’re interested in additional psychology only sessions.
